The State Bar of Michigan has created a "Request for Accommodations" form for use by individuals to make
sure they receive open access to the judicial system. It can be used by people who have been called for jury duty or it is for people
who are involved in the court process as witnesses or as part of civil criminal litigation. The person making the request should be
very specific about the type of accommodation needed. This form can be obtained from the Michigan Supreme Court State Administrators
office or the
Michigan Courts website. Click "Court Forms". Enter "MC 70" in the search box.
